form post into feedback tpl

heya, I want to use a form select value into my feedback template, but it's not a collector and the form redirects to /content/collectedinfo/...

how can I use this additional form info?

my solution:

I add a selecttion list to my feedback form class and use this id in template like

<select size="1" name="ContentObjectAttribute_ezselect_selected_array_6253[]">
