Editor is notified Editor reviews content Editor approves content Staff member is notified that content is published. It appears that workflows and collaboration don't work from initial set up despite following the documentation.

How do I get this scenario to work:

Staff member creates content.
Editor is notified
Editor reviews content
Editor approves content
Staff member is notified that content is published.
