Size in icon.ini.append.php doesn't work

If Theme=crystal-admin then I have to put kmidi.png in icons/crystal-admin/16x16_indexed/apps/ and if Theme=crystal then I have to put kmidi.png in icons/crystal/16x16/apps/kuser.png I tried several syntaxes for Size but nothing works. Evrery thing works fine but not Size.

If Theme=crystal-admin
then I have to put kmidi.png in icons/crystal-admin/16x16_indexed/apps/
and if Theme=crystal
then I have to put kmidi.png in icons/crystal/16x16/apps/kuser.png

I tried several syntaxes for Size but nothing works. I don't understand which file handles the Size
