extension & overriding pagelayout.tpl

I need to append one tab to admin page menu.

Is it possible to do so (override pagelayout.tpl) only with extension or do I have to change pagelayout.tpl directly?

It would be nice, if it's possible to merge original admin site design with several my extesion templates.
