An extension development problem.

Hi, all

I am using the eZpublish 3.6. In my site, I want to develope an extension system for creating one form namely "CV", after filling this form when user send, that information i.e. Name and Prename etc, to be seen in the back office.

For creating this extension, I also refer & try this following link: http://ez.no/ez_publish/documentation/development/extensions
As it is too useful but I could not solve my purpose, so can anybody guide me?

Probably you are right, but for maintenance purpose, I want to do this using extension so if you can say how can I build this through extension then I will be very thankful.
