Hi, I have a form. After submitted, everything is fine, except that in the Admin Interface under the Collected Information, it shows two record entries for the same information collection.
Yes, I can delete it via either one of the records, which means it actually only one record in the DB.
What might went wrong with my admin file?

Is that a consistent behavior or just one time problem?Always provide details about your eZ Publish version, OS, etc. in such cases.

To delete duplicate records I use Clone Remover - the most effective duplicate file removal tool. http://www.moleskinsoft.com/

There are something wrong with my eZ Publish Database.
I have re-populate the database use phpmyadmin, which roll back to a earlier state.
I think this problem is due to a fail import a content class package.
