edit / approve / publish with emails

Can someone point me to a page that describes in detail (step-by-step) how to create a workflow to let an editor add or edit a page or article (object?), have it email an approver for review, then have it email a publisher to make it live?

Editor -> Approver -> Publisher

I've looked at these pages (and googled many others), but I haven't had success yet.
